CLASSES WILL START January 20th, 2010!!!!
(Winter/Spring 2010 semester will have a 16 week semester for all HIGH SCHOOL and some Junior High students-all others will have a 12 week semester)
Please click on the navigation bar to the left to view and print all 3 registration forms!! When mailing your registration, please COMPLETE ALL INFORMATION (including ages of children) and send these THREE forms:
*Registration form
*Medical Release (unless already filled out for last semester)
*Liability Form (unless already filled out for last semester)
along with the $30 REGISTRATION FEE PER FAMILY PER SCHOOL YEAR and 1/2 of the tuition due for each child made payable to HELP.
***IF, AT ANY TIME, YOU ARE EXPERIENCING A PROBLEM WITH THESE FORMS AND NEED THEM EMAILED TO YOU AS AN ATTACHMENT, PLEASE EMAIL KIMBERLY CONLIN AT KCONLIN@CINCI.RR.COM !!
***PLEASE NOTE: If you would like to send in PAYMENT IN FULL, we will gladly accept it, as this helps us with paperwork processing (SOOO much easier!!!). We CANNOT confirm your child's spot until ALL these forms are in!!!
