Registration form/Fees
REGISTRATION FOR WINTER/SPRING 2012 CLASSES 
for ALL families is now OPEN!

2nd Semester Classes began on Wednesday, January 11th, 2012.

THESE CLASSES ARE ALREADY FULL
for the 2012 Spring/Winter Semester:

Junior Literature
IEW at 1:00 pm
Literature & Composition for 7th/8th grades
Art at 1:15
Art at 3:30
Music Theatre Junior AND Senior
Beginning Dance class
Group Voice at 10:30 am



To register for any class, follow these steps:

1.  Complete three forms (links on menu to left)
   
    :  Registration Form
    :  Medical Release Form
    :  Liability Form

      Incomplete forms cannot be processed, so be sure to fill in all required data.

NOTE:  If you have difficulty accessing any form, please contact Kimberly Conlin Kim.thecollinsgroup@gmail.com and they will be sent to you via email attachment.


2.  Write a check for your fees.
    :  Include the $30.00 Registration Fee (per family, per school year)
    :  Calculate your total tution and submit 50%.
 
     All checks should be made payable to:  HELP


NOTE: Full tuition payment is accepted.  Partial payment is merely offered as a convenience for HELP families.


3.  Submit fees and all three completed forms.

Be advised that class registration is given on a first come, first saved basis. A student's class selection cannot be secured until all of the required paperwork and fees are received by the administration.  Class confirmation will then be provided.
